A Case of Ixekizumab-induced Paradoxical Palmoplantar Pustulosis Successfully Treated with Guselkumab

Abstract
Paradoxical reaction refers to the appearance or worsening of pathological responses during treatment with biologics. Pathological responses appear in pre-existing lesions or in other forms, and palmoplantar pustulosis (PPP) is one of the most common forms. Most biologics that cause paradoxical reactions are tumor necrosis factor (TNF) blockers. Recently, new biologics have been released, and paradoxical reactions by these biologics have also been reported. A 59-year-old woman with a 19-year history of plaque psoriasis started treatment with ixekizumab. After 4 months of treament PASI 100 was achieved and well controlled. However the patient presented progressive worsening of pustular lesions on the palms and soles. The patient was diagnosed as PPP on histopathological examination and improved after switching to guselkumab.
